
Testicular Pain, Swelling or Heaviness
When it is an emergency and when it is examined.
Read this first — an emergency
Sudden, severe testicular pain, particularly with nausea or rapid swelling, may be testicular torsion.
This is an emergency measured in hours rather than days, and every hour of delay reduces the chance of saving the testicle.
Do not wait for an appointment and do not try a painkiller — go to the nearest emergency department immediately.
What is the problem?
Pain, swelling or a feeling of heaviness in the testicle can arise from causes that differ entirely in seriousness and in treatment: infection, a collection of fluid, dilated veins around the testicle known as a varicocele, or other causes.
These causes cannot be distinguished by touch or by description. The difference between them appears in blood flow — and that is what Doppler measures.
Varicocele — the symptom many recognise but cannot name
A sense of heaviness or dragging in the scrotum rather than sharp pain.
It increases after long standing or at the end of the day, and eases on lying down.
It appears on the left side more often than the right.
It may be discovered while investigating delayed conception, because varicocele is among the most common treatable causes in men.

Testicular Doppler ultrasound
Imaging that shows the structure of the testicle and the blood flow within it. It is the investigation that distinguishes infection from varicocele from a fluid collection, and measures the degree of venous dilatation. It requires no anaesthetic, causes no pain, and is performed inside the centre.
Which tests are usually requested?
The tests here depend on the reason for the visit — pain and infection, or delayed conception:
- Semen analysis
- The core investigation when the reason for the visit is delayed conception. It is sometimes repeated after an interval to confirm the result.
- Urinalysis and culture
- Where infection of the testicle or epididymis is suspected.
- Fertility hormones and testosterone
- Requested where the semen analysis is abnormal, to locate the source of the problem.
An important exception
Sudden severe pain does not wait for a test or an appointment.
Suspected testicular torsion is managed with imaging and emergency care immediately — tests come after that, not before.
What you leave the visit with
A specific diagnosis rather than a general description — what kind of problem it is, what grade, and whether it needs intervention or monitoring only.
